The ALJ's initial order must:

1. (1) Be correctly captioned, identifying DCYF, the name of the proceeding, and the docket number;
2. (2) List the names of all parties and representatives who participated in the proceeding;
3. (3) Contain numbered findings of fact based on the evidence in the record or officially noticed in the proceeding that the ALJ relied on in coming to a decision;
4. (4) Identify and explain findings based substantially on credibility of evidence or on demeanor of witnesses;
5. (5) Contain numbered conclusions of law and the reasons and basis for them, including citations of statutes and rules relied upon;
6. (6) Contain an order disposing of all contested issues, stating the result and remedy ordered;
7. (7) Contain a statement describing available procedures and time limits for requesting changes to the initial order or review by the BOA;
8. (8) State when the decision becomes final; and
9. (9) Include any other information required by law or DCYF program rules.
