1. A. The provider shall report to the department within 24 hours of the circumstances surrounding the following incidents:

   1. 1. Lost or missing child when local authorities have been contacted for help;
   2. 2. Serious injury to a child while under the family day home's supervision; and
   3. 3. Death of a child while under the family day home's supervision.
2. B. A written report shall be completed and submitted to the department within five working days of the date the incident occurred.
