1. A. The provider shall record in the child's record an injury or accident sustained by a child while at the family day home that requires first aid or emergency medical or dental treatment.

2. B. The information recorded shall include the following:

   1. 1. Date and time of injury;
   2. 2. Name of injured child;
   3. 3. Type and circumstance of the injury;
   4. 4. Caregiver present and action taken;
   5. 5. Date and time when parents were notified;
   6. 6. Any future action to prevent recurrence of the injury;
   7. 7. Caregiver and parent signatures or two caregiver signatures; and
   8. 8. Documentation on how the parent was notified.
