1. A. A physician assistant whose license is lapsed for a period of two years or less may reinstate the license by payment of the renewal and late fees as set forth in 18VAC85-50-35.

2. B. A physician assistant whose license is lapsed for a period of more than two years may apply for reinstatement and shall submit:

   1. 1. A completed application package;
   2. 2. The fee specified in 18VAC85-50-35; and
   3. 3. Evidence of current certification by NCCPA.
