1. 1. Cause. There must exist reasonable cause for revocation prior to such action by the commissioner.
2. 2. Notification. The Office of EMS must notify the grantee of the revocation by certified mail to the last known address.
3. 3. Period of effect. A revocation shall be permanent unless and until overturned on appeal.

The commissioner may revoke an award and all disbursements of funds attached after an informal fact-finding conference as defined in the Administrative Process Act (§ 2.2-4000 et seq. of the Code of Virginia) or waiver thereof.
