1. A. The provider shall maintain the following emergency medical information for each individual:

   1. 1. If available, the name, address, and telephone number of:

      1. a. The individual's physician; and
      2. b. The authorized representative or other person to be notified;
   2. 2. Medical insurance company name and policy or Medicaid, Medicare , or CHAMPUS number, if any;
   3. 3. Currently prescribed medications and over-the-counter medications used by the individual;
   4. 4. Medication and food allergies;
   5. 5. History of substance abuse;
   6. 6. Significant medical problems or conditions;
   7. 7. Significant ambulatory or sensory problems;
   8. 8. Significant communication problems; and
   9. 9. Advance directive, if one exists.
2. B. Current emergency medical information shall be readily available to employees or contractors wherever program services are provided.
