1. A. The provider shall develop a method for documenting the provision of crisis intervention and emergency services. Documentation shall include the following:

   1. 1. Date and time;
   2. 2. Description of the nature of or circumstances surrounding the crisis or emergency;
   3. 3. Name of individual;
   4. 4. Description of precipitating factors;
   5. 5. Interventions or treatment provided;
   6. 6. Names of employees or contractors responding to or consulted during the crisis or emergency; and
   7. 7. Outcome.
2. B. If a crisis or emergency involves an individual who is admitted into service, documentation of the crisis intervention or provision of emergency services shall become part of his record.
