1. A. Application for account wagering license.

   1. 1. A nonrefundable application fee of $5,000 must be submitted with the application.
   2. 2. The applicant will be billed and shall be responsible for any costs involved in the background checks and review of the application in excess of $5,000.

2. B. Annual fees.

   1. 1. An annual fee of $1,000 shall be payable to the commission on issuance of the original license and thereafter on or before January 1 of each year. The licensee will be billed and shall be responsible for any costs in excess of $1,000 associated with the review of the annual renewal application.
   2. 2. A license fee of one and one-half percent (1.5%) of the gross total handle from the source market area shall be payable to the commission on the tenth day of each month for the previous month.
