1. 1. The specific dates;
2. 2. The breed of horse;
3. 3. The host track or tracks;
4. 4. The number of races;
5. 5. The number of races per date; and
6. 6. Any other information the licensee deems appropriate in assisting the commission in evaluating the disclosure.

If the simulcasting dates are requested from a location other than those emanating from the licensee's racetrack, the licensee shall disclose, to the extent known, the simulcast schedule including:
