1. (1) verifies that the sole practitioner who is renting or leasing is a licensed massage therapist and in good standing in the state of Utah at the time of the rental or lease;
2. (2) obtains a signed attestation from the sole practitioner who is renting or leasing that the sole practitioner has no business arrangement with the licensed individual other than a rental or lease; and

3. (3) produces copies of the following if requested by the division:

   1. (a) the sole practitioner's state massage license;
   2. (b) the sole practitioner's signed attestation described in Subsection (2); and
   3. (c) the rental agreement.
