1. (1) Without specific authorization from a prescriber, a pharmacist or pharmacy intern may dispense:

   1. (a) a prescription in a quantity different than the quantity prescribed if the prescribed quantity or package size is not commercially available; and
   2. (b) a prescription in a dosage form different than the dosage form prescribed, if in the professional judgement of the pharmacist or pharmacy intern, dispensing a different dosage form is in the best interest of the patient.

2. (2) This section does not apply if:

   1. (a) the substitute would change the bioavailability of the medication;
   2. (b) the substitute would change the treatment parameters; or
   3. (c) the prescriber has written or clearly designated "dispense as written" on the prescription.
