1. (1) An applicant for licensure as a pharmacy technician trainee shall:

   1. (a) submit an application to the division on a form created by the division;
   2. (b) pay a fee established by the division in accordance with Section 63J-1-504;
   3. (c) unless exempted by the division, submit a completed criminal background check;
   4. (d) demonstrate, as determined by the division, that the applicant does not have a physical or mental condition that would prevent the applicant from engaging in practice as a pharmacy technician with reasonable skill, competency, and safety to the public;
   5. (e) submit evidence that the applicant is enrolled in a training program approved by the division; and
   6. (f) satisfy any other criteria established by division rule made in collaboration with the board.
2. (2) A pharmacist whose license has been denied, revoked, suspended, or restricted for disciplinary purposes is not eligible to be licensed as a pharmacy technician trainee during division probation.
