1. (1) This Subsection R746-1-202 does not apply to complaints.

2. (2) A person that files a pleading shall include the following information in the title:

   1. (a)(i) name and bar number of attorney preparing the pleading; or

      1. (ii) if no attorney is involved, name of the person signing the pleading;
   2. (b) address, telephone number, and e-mail address of the person identified in Subsection R746-1-202(2)(a);
   3. (c) nature of the request;
   4. (d) description of the action or relief requested;
   5. (e) type of pleading; and
   6. (f) docket number, if known.
