1. (1) A county jail applying for the funding of suicide barriers shall submit the application to the department:

   1. (a) on a form provided by the department; and
   2. (b) submitted no later than May 1 each year.

2. (2) In order receive awarded funds for the purchase of equipment, the law enforcement agency shall submit to the department a completed application form that includes:

   1. (a) criteria listed in Subsection 17-22-34(3)(a); and
   2. (b) the total amount of funding being requested.
