1. A. The department may, on the recommendation of the program supervisor, revoke or suspend the certification of any operator or technician:

   1. (1) Who fails to comply with or meet any of the criteria required in this rule; or
   2. (2) Who falsely or deceitfully obtained certification; or
   3. (3) Who fails to show proficiency in proper operation of the breath testing instrument; or
   4. (4) For other good cause.
