1. (1) The required fee will accompany the application for license or certificate of registration.

   1. (a) License or certificate of registration fees will be refunded if the application is denied.

2. (2) When a certificate of registration has expired for more than one year, an application will be made for an original certificate as if the application was being made for the first time.

   1. (a) Examinations will be re-taken with initial fees.
