1. (1) A requester shall:

   1. (a) provide acceptable proof of identification including:

      1. (i) driver license;
      2. (ii) identification card; or
      3. (iii) other official documentation determined by the division;
   2. (b) declare one or more permissible uses within the Driver's Privacy Protection Act of 1994 under which the requester is qualified to receive the information;
   3. (c) provide sufficient information to locate the MVRs;
   4. (d) pay appropriate fees in a manner approved by the division; and
   5. (e) agree to comply with state and federal laws regulating resale and further disclosure of information on an MVR.
2. (2) The division may determine the requester is not entitled to receive an MVR if the division has reason to believe the declaration is invalid, or that any other condition in this rule has not been met.
