1. (1) Each person who is employed by a school to perform administrative duties, and requires authorization to access DEMS for the school, shall submit an application:

   1. (a) on a form approved by the division;
   2. (b) signed by the applicant;
   3. (c) signed by the owner of the school as verification of employment by the school; and
   4. (d) accompanied by a fingerprint card for a criminal background check and payment for the cost associated with the criminal background check.
2. (2) Each commercial school will be responsible to train personnel on DEMS.
