1. (1) An individual requesting an alcohol or drug hearing shall:

   1. (a) submit a request in writing to the division within ten calendar days of the notice of arrest; and

   2. (b) include the driver's:

      1. (i) full name;
      2. (ii) date of birth;
      3. (iii) date of arrest;
      4. (iv) current address;
      5. (v) Utah license number if applicable; and
      6. (vi) a designation if the driver will attend the hearing in person at a division office, telephonically, or by live audiovisual means.

2. (2) An individual making a request under Subsection (1) may submit the request to the division by:

   1. (a) accessing the division's website dld.utah.gov/dui-hearing-request/;
   2. (b) emailing dldui@utah.gov; or
   3. (c) mailing the written request to PO Box 144501, Salt Lake City, Utah 84114-4501.
