1. (1) The parole agent shall, on a form approved by the Board, provide the Board with the following information:

   1. (a) the parolee's risk and need assessment level at the time of the current violation and a summary of the areas of concern;
   2. (b) the number of prior paroles;
   3. (c) the parolee's parole violation history;
   4. (d) the parolee's custody status;
   5. (e) financial obligation details regarding the parolee;
   6. (f) the parolee's address or living arrangements;
   7. (g) the parolee's treatment summary;

   8. (h) the results of any drug or alcohol tests;

      1. (i) any new referred offenses or new criminal charges;
   9. (j) any aggravating factors concerning the parolee;

   10. (k) any mitigating factors concerning the parolee; and

       1. (l) a summary of the parolee's current parole performance.
