1. (1) Pursuant to Subsection 4-41a-1005(1), the department shall divide the state into geographic regions based on:

   1. (a) overall population;
   2. (b) patient population; and
   3. (c) travel time.

2. (2) The department establishes the following geographic regions by county:

   1. (a) Region 1- Northern Utah:

      1. (i) Box Elder;
      2. (ii) Cache; and
      3. (iii) Rich.

   2. (b) Region 2 - Weber Basin

      1. (i) Davis;
      2. (ii) Morgan; and
      3. (iii) Weber;

   3. (c) Region 3 - Salt Lake:

      1. (i) Salt Lake;
      2. (ii) Summit; and
      3. (iii) Tooele;

   4. (d) Region 4 - Uintah Basin:

      1. (i) Daggett;
      2. (ii) Duchesne; and
      3. (iii) Uintah;

   5. (e) Region 5 - West Central:

      1. (i) Juab;
      2. (ii) Sanpete;
      3. (iii) Utah; and
      4. (iv) Wasatch;

   6. (f) Region 6- Central:

      1. (i) Carbon;
      2. (ii) Emery;
      3. (iii) Millard;
      4. (iv) Piute;
      5. (v) Sevier; and
      6. (vi) Wayne;

   7. (g) Region 7 - Southwest:

      1. (i) Beaver;
      2. (ii) Garfield;
      3. (iii) Iron;
      4. (iv) Kane; and
      5. (v) Washington; and

   8. (h) Region 8 - Southeast:

      1. (i) Grand; and
      2. (ii) San Juan.
