1. (1) To obtain a grant under this rule, an applicant shall submit an application to the division on or before the application deadline date established by the division.
2. (2) Each applicant shall apply to the division in writing upon forms available from the division.

3. (3) An application submitted to the division shall:

   1. (a) be signed and dated by an authorized representative of the applicant; and

   2. (b) include the following information:

      1. (i) a detailed cost estimate of the proposed project study, design, or construction prepared by a licensed professional engineer;
      2. (ii) a map showing the location of the proposed project;
      3. (iii) an estimated timeline for the project, including the projected start date and expected completion date;
      4. (iv) applicant's verification that the information provided in the application is accurate and any estimates or projections submitted are based on sound professional judgment and the best available data; and
      5. (v) additional information required on the application.
