1. (1) An agency title insurance producer shall file a Title Insurance Producer Annual Report not later than May 1 of each year.

2. (2) A Title Insurance Producer Annual Report shall include:

   1. (a) the number and location of each title or escrow trust account;
   2. (b) proof of financial protection under Subsection 31A-23a-204(2)(a);
   3. (c) the name of the individual title insurance producer designated as the "qualifying licensee" under Subsection 31A-23a-204(1);
   4. (d) the physical address in Utah maintained by an agency title insurance producer;
   5. (e) the physical address of each Utah branch office maintained by an agency title insurance producer;

   6. (f) a verified statement of the agency title insurance producer's financial condition, transactions, and affairs of the preceding two calendar years, including:

      1. (i) a balance sheet;
      2. (ii) a cash flow statement; and
      3. (iii) an income statement; and
   7. (g) a list of all currently contracted underwriters.
