1. (1)(a) An applicant shall promptly notify the commissioner of any change in the information furnished on the Form A arising after the date the applicant furnished the information to the commissioner.

   1. (b) The applicant shall notify the commissioner of the change in information prior to the commissioner's final decision regarding the application.
