1. (1) The purpose of this rule is to establish a universal application for all insurers offering a health benefit plan.

2. (2) This rule applies to:

   1. (a) an individual health benefit plan offered outside the Federally Facilitated Marketplace; and
   2. (b) a small employer health benefit plan.
