1. (1) The purpose of this rule is to:

   1. (a) identify each county with a population density of less than 100 people per square mile;
   2. (b) identify independent hospitals;
   3. (c) identify federally qualified health centers; and

   4. (d) describe how a managed care organization:

      1. (i) uses the information described in Subsections (1)(a), (b), and (c);
      2. (ii) notifies enrollees, independent hospitals, and federally qualified health centers of the information described in Subsections (1)(a), (b), and (c); and
      3. (iii) provides notice required by Subsection 31A-45-501(8)(d)(ii).
2. (2) This rule applies to a managed care organization.
