1. (1) If the division becomes aware that a provider is in violation of these rules or other rules or statutes referenced in these rules:

   1. (a) Within 30 days after becoming aware of the violation, the division shall identify in writing the specific areas in which the provider is not in compliance and send written notice to the provider.
   2. (b) Within 30 days of notification of noncompliance, the provider shall submit a written plan for achieving compliance. The provider may be granted an extension.
