- (1) a seminar provider or applicant violates this rule or Section 26B-5-205;
- (2) an applicant fails to correctly complete each required step of the application process; or
- (3) a provider whose certification has been previously denied, suspended or revoked has reapplied without completing a previously required corrective action plan.
The division may deny, suspend or revoke certification if:
