- (1) As part of the evaluation of capacity for ongoing care of a child, in addition to background screening required for preliminary placement, anyone over the age of 18 years in the home must complete an FBI fingerprint-based criminal history records check. Background screening for placement with the non-custodial parent is subject to Sections 80-3-302, 80-3-303, and 80-3-305, or court order.
- (2) A Utah child abuse registry check will be completed for persons over the age of 18 years residing in the home. If any person 18 years of age or older residing in the home has lived out of Utah in the five years immediately preceding the date of the application, a child abuse and neglect registry check must be completed for any state in which the individual resided.
(3) Persons 18 years of age and older living in the household must provide the following information on a form provided by Child and Family Services in order for background screening to be conducted:
- (a) full first, middle, last, maiden, alias, and previous married names;
- (b) Social Security number, if a number has been issued;
- (c) proof of identity verified by a government-issued photo identification;
- (d) date of birth;
- (e) the potential kinship caregiver and applicable adults living in the household shall provide fingerprints from an authorized law enforcement agency or designated electronic scanning site; and
- (f) if the applicant has lived outside of Utah in the five years preceding the date of the application, a list of the states the applicant has lived in must be provided.
