Utah Admin. Code R436-5-4
(1)(a) A child whose parentage has been determined by court order, or their legal representative, may have their birth registration information updated and obtain a supplementary birth certificate by presenting a certified copy of the court order to the state registrar along with the required fee. The state registrar shall update the birth registration information and prepare the supplementary birth certificate with the full name of the child as specified in the court order.
(2) If the court order does not specify the name to be placed on the supplementary birth certificate and if the current birth certificate does not list the name of the child, the state registrar shall:
