(1) The Office of Vital Records and Statistics (office) shall include on the foundling certificate form provided by the office:
(a) information from the hospital that receives an infant of unknown parentage found in the state, including:
- (i) the date and address of the place of finding, which will be used as the place of birth;
- (ii) the sex, approximate live birth date, and race of the infant if known;
- (iii) the name and address of the person or institution submitting the report; and
- (iv) other medical information, if available; and
- (b) the name for the infant obtained from the Division of Child and Family Services.
- (2) Information submitted under this section shall constitute the basis for the report of live birth for the infant.
