1. (1) A registrant shall:

   1. (a) be at least 18 years old;
   2. (b) complete registration online through the adoption registry website or submit a paper form provided by the office; and
   3. (c) pay the registration fee, which is required regardless of whether a match is made.

2. (2) Each registrant on the adoption registry shall include, to the fullest extent known:

   1. (a) the registrant's current name;
   2. (b) the registrant's current contact information, including an email address;
   3. (c) the birth mother's name at time of the adoptee's birth;
   4. (d) the birth father's name at the time of the adoptee's birth;
   5. (e) the birth mother's current name;
   6. (f) the birth father's current name;
   7. (g) the adoptee's birth name;

   8. (h) the adoptee's adoptive name;

      1. (i) the adoptee's current name;
   9. (j) the adoptee's birth date;

   10. (k) the adoptee's birth place, including the city, county, and facility or location of the birth;

       1. (l) the name of each adoptive parent at the time of adoption;
   11. (m) the name of the adoption agency or private attorney that handled the adoption; and
   12. (n) any additional information that will help to identify the birth parent or adoptee.

3. (3) The registrant may elect to:

   1. (a) receive identifying information for another registrant;
   2. (b) share registrant identifying information, which may include a method for contact;
   3. (c) share registrant identifying information but decline to provide a method for contact; or
   4. (d) share name and contact information of an intermediary who will facilitate communication between the matched registrants.
4. (4) A birth parent may allow the office to make available for inspection by an adoptee other information about the birth parent, including an updated health history.
5. (5) A birth parent may prohibit the office from sharing any of that birth parent's information or documents with the adoptee if the birth parent gets a court order sealing the records in accordance with Section 81-13-103.

6. (6)(a) An adoptee may request a non-certified copy of the adoptee's original birth certificate.

   1. (b) An adoptee may request other non-identifying information, including an updated medical history and contact information provided by the birth parent.
