1. (1) the extent to which the applicant:

   1. (a) demonstrates that it will provide assistance to the greatest number of persons with bleeding disorders residing across the state;
   2. (b) utilizes other sources of funding, including private funding, to provide bleeding disorder services; and

   3. (c) provides:

      1. (i) information that meets the requirements established in Subsection 26B-4-324(3);
      2. (ii) a description of the individuals to be served by the grant;
      3. (iii) the estimated number of individuals to be served with the grant award; and
      4. (iv) the results of an assessment of need demonstrating the need for the bleeding disorder services that the grantee proposes to provide;
2. (2) the cost to the person with a bleeding disorder for the bleeding disorder services;
3. (3) the degree to which the applicant meets the requirements of the statute; and
4. (4) the degree to which the application is feasible, clearly described, and ready to be implemented.
