- (1) "Department" means the Department of Health and Human Services.
(2) "Equipment" means capital equipment that:
(a)(i) costs at least $5,000; or
- (ii) is a group of items that each individually cost less than $5,000 but, when combined, make up one functional unit with a combined cost of at least $5,000;
- (b) has a life span of at least three years;
- (c) is non-expendable material; and
- (d) is not consumed.
- (3) "Innovative" means aspects that are new, different, or more efficient, while also providing significant benefit to the community and the underserved populations served by the project.
- (4) "Office" means the Office of Primary Care and Rural Health, within the Division of Population Health under the department.
- (5) "Sustainable" means a project or service that can be continued without Primary Care Grant Program funds.
(6) "Underinsured" means an individual:
- (a) with public or private insurance that does not cover necessary health care services, resulting in out- of-pocket expenses that exceed the individual's ability to pay;
- (b) who is denied a full coverage plan from work; have;
- (c) who has a health insurance plan which only covers the worker and not the family or extended family; or
- (d) who has a health insurance plan with a high deductible or co-insurance.
(7) "Uninsured" means an individual who:
- (a) lacks public or private insurance; and
(b) is unable to afford health insurance; or
- (ii) is denied paid health care through the individual's employer.
