1. (1) performance;
2. (2) commitment to the medically underserved area;
3. (3) service obligation fulfillment; and
4. (4) any other information regarding experience under the Conrad 30 J-1 visa waiver as the department determines is reasonably necessary for the administration of the program.

The department may require the health care facility and Conrad 30 J-1 visa waiver physician to provide information regarding:
