1. (1) The agency shall obtain a signed and dated service agreement from the client and/or his responsible party. The service agreement shall include the following:

   1. (a) A description of services to be performed by the Personal Care Aide;
   2. (b) Charges for the services;
   3. (c) A statement that a 30-day notice shall be given prior to a change in base charges.
