1. (1) If construction documents are not available for existing buildings or for facilities requesting an initial license or license category change, the applicant or licensee may submit to OL the following information:

   1. (a) a functional program described in Section R432-4-9; and
   2. (b) a report identifying required modifications to bring the building into compliance with each applicable code or construction rule for the requested licensure category.
2. (2) The applicant or licensee shall request and schedule an OL follow-up inspection upon completion of modifications.
