1. (1) The licensee must ensure a signed and dated resident assessment is completed for each resident before admission and at least every six months thereafter.
2. (2) In each type I and type II assisted living facility, a licensed health care professional shall complete and sign the initial and six-month resident assessment.

3. (3) The licensee shall ensure that the resident assessment:

   1. (a) accurately reflects the resident's status when assessed; and
   2. (b) includes a statement signed by the licensed health care professional completing the resident assessment that the resident meets the admission and level of assistance criteria for the facility.
4. (4) The licensee shall ensure the resident assessment form is approved by the OL for documenting resident assessments.

5. (5)(a) The licensee shall ensure each resident's assessment is revised and updated when there is a significant change in the resident's:

   1. (i) cognitive condition;

      1. (ii) medical condition;
      2. (iii) physical condition; or
      3. (iv) social condition.
   2. (b) The licensee shall update the resident's service plan to reflect any change in condition.
