1. (1) The licensee shall ensure the following regarding resident care plans:

   1. (a) a written resident care plan, coordinated with nursing and other services, is initiated for each resident upon admission;

   2. (b) the resident care plan is personalized and indicates:

      1. (i) measurable and time-limited objectives;
      2. (ii) the plan of care; and
      3. (iii) the professional discipline responsible for each element of care;
   3. (c) the resident care plan is developed, reviewed, revised, and updated at least annually through documented conferences with any professionals involved in the resident's care;
   4. (d) each resident's care is based on their individualized plan;
   5. (e) the resident care plan is available to any personnel who care for the resident;
   6. (f) the resident and family shall participate in the development and review of the resident's plan;
   7. (g) upon transfer or discharge of the resident, relevant information from the resident care plan is made available to the responsible institution or agency; and
   8. (h) a licensed nurse or other clinical specialist, where appropriate, completes a monthly summary of the resident's status and problems identified in the resident care plan.

2. (2) The licensee shall ensure that each resident care plan shall include the following:

   1. (a) name, age, and gender of resident;
   2. (b) diagnosis, symptoms, and resident complaints;
   3. (c) a description of the functional level of the individual;
   4. (d) care objectives and time frames for accomplishment, reevaluation, and completion;
   5. (e) discipline or person responsible for each objective;
   6. (f) discharge plan;
   7. (g) date of admission; and
   8. (h) name of attending physician or medical practitioner.
