1. (1) The licensee shall comply with Surgical Services, Subsections:

   1. (a) R432-100-16(1)(a) through (e); and
   2. (b) R432-100-16(1)(g) through R432-100-16(4).

2. (2) The licensee shall comply with the following sections of Rule R432-100:

   1. (a) Rehabilitation Therapy Services, R432-100-22;
   2. (b) Outpatient Services, R432-100-30; and
   3. (c) Pediatric Services if provided, R432-100-20.
