1. (1) "Anterior tooth" means tooth numbers:

   1. (a) 6 through 11;
   2. (b) 22 through 27;
   3. (c) C through H; and
   4. (d) M through R.
2. (2) "Dental services" whether furnished in a hospital, an office, a skilled nursing facility, or anywhere else, means covered services performed within the scope of a Medicaid-enrolled dental provider's license as defined in Title 58, Occupations and Professions.

3. (3) "Posterior tooth" means tooth numbers:

   1. (a) 1 through 5;
   2. (b) 12 through 21;
   3. (c) 28 through 32;
   4. (d) A through B;
   5. (e) I through L; and
   6. (f) S through T.
