HSCT Transplantation, Non-Covered Services

(1) HSCT is not covered as treatment for multiple myeloma.
(2) AuSCT is not covered for:
1. (a) Acute leukemia not in remission;
2. (b) Chronic granulocytic leukemia;
3. (c) Solid tumors (other than neuroblastoma);
4. (d) Tandem transplantation (multiple rounds of AuSCT) for patients with multiple myeloma;
5. (e) Non-primary AL amyloidosis; or
6. (f) Primary AL amyloidosis for patients who are at least 64 years of age.
(3) All other conditions not specifically listed as covered in this rule.
