1. (1) if live born, the child's last name;
2. (2) if live born, the child's first name;
3. (3) the child's date of birth;
4. (4) the child's medical record number;
5. (5) the child's gender;
6. (6) the mother's last name;
7. (7) the mother's first name;
8. (8) the mother's maiden name;
9. (9) the mother's date of birth;
10. (10) the mother's medical record number;
11. (11) the delivery institution;
12. (12) the ICD - 10 birth defect codes;
13. (13) the mother's state of residency at delivery; and
14. (14) the mother's zip code of residency at delivery.

Each hospital, clinic, institution, or birthing center that admits a patient and detects or screens for a birth defect as a result of any outcome of pregnancy, or admits a child under 24 months of age with a birth defect, or is presented with the event of a stillbirth shall report or cause to report to the department within 40 days of discharge the following:
