1. (1) This rule implements the immunization requirements of Title 53G, Chapter 9, Part 3 Immunization Requirements. It establishes minimum immunization requirements for attendance at a public, private, elementary, or secondary school through grade 12, nursery school or kindergarten, licensed day care center, child care facility, family home care, or Head Start program in this state. It establishes:

   1. (a) required doses and frequency of vaccine administration;
   2. (b) reporting of statistical data; and
   3. (c) time periods for conditional enrollment.
2. (2) Section 53G-9-305 authorizes this rule.
