1. (1) Each reporting entity shall provide the department or local health department with any records or other material requested by the department and necessary to conduct a thorough investigation, including:

   1. (a) medical records;
   2. (b) additional laboratory testing results;
   3. (c) treatment and vaccination history;
   4. (d) clinical material; or
   5. (e) contact information for any case, suspect case, or person potentially exposed.
2. (2) Any individual or entity shall grant the department or local health department on-site access to a facility when the department or local health department decides such access is critical to a public health investigation.
