1. (1) To be eligible to receive services from the Breast and Cervical Cancer Screening Program, an individual must:

   1. (a) be at least 21 years old;
   2. (d) be a current Utah resident;
   3. (b) have income at or below 250% of federal poverty level; and
   4. (c) have no insurance or inadequate insurance coverage that does not pay for these services.

2. (2) To be eligible to receive services from the BeWise Program, an individual must:

   1. (a) be enrolled and remain eligible to participate in the Breast and Cervical Cancer Screening Program;
   2. (b) have income at or below 250% of the federal poverty level;
   3. (c) have no insurance or inadequate insurance coverage that does not pay for these services; and
   4. (d) not be enrolled in Medicare Part B.
