1. (1) The following content shall be included in each application for an Economic Assistance Grant:

   1. (a) name of applying Business Entity;
   2. (b) tax ID;
   3. (c) amount of grant funding requested;

   4. (d) responsible contacts':

      1. (i) name;
      2. (ii) full mailing address;
      3. (iii) telephone number; and
      4. (iv) email address;
   5. (e) a description of the project and the activities the funds will be used for;
   6. (f) a description of expected deliverables and outcomes;
   7. (g) a description of matching funds, if any:

   8. (h) provided by any one or combination of:

      1. (A) a private sector entity;
      2. (B) a nonprofit entity;
      3. (C) a federal matching grant; or
      4. (D) any other funding source approved by the office; and
   9. (i) any other information requested by the office.

2. (2) The following documentation shall be included in each application for an Economic Assistance Grant:

   1. (a) the entity's W9 form;
   2. (b) budget describing proposed uses of grant funds;
   3. (c) prior year and current year operating budgets; and
   4. (d) any other documentation requested by the office.
