1. (1) evidence that the county's unemployment rate was at least 5% for the six consecutive months before the application date;
2. (2) evidence that the county experienced year over year economic decline; and
3. (3) other evidence as requested by the office.

To establish that a county is experiencing economic distress a business entity or county shall submit to the office:
