1. (1) The content of the application for a rural employment expansion grant shall, at minimum, include the business entity's:

   1. (a) name;
   2. (b) physical operating address;
   3. (c) telephone number;
   4. (d) email address;
   5. (e) Federal EIN number;
   6. (f) primary NAICS code;
   7. (g) vendor number, if the applicant is a registered vendor with the state;

   8. (h) requested rural employment expansion grant amount; and

      1. (i) forecasted:
      2. (i) number of new, full-time employee positions; and
      3. (ii) wage of new, full-time employee positions.

2. (2) The following documents shall, at minimum, be included in each application for participation in the program:

   1. (a) a copy of a current, signed W-9 form;

   2. (b) evidence of the business entity's employees during the baseline period such as:

      1. (i) the four most recent Form 33H - Utah Employer Quarterly Wage List and Contribution Reports;
      2. (ii) a copy of an executed professional employee agreement for each employee, as defined in Subsection 31A-40-102(15); or
      3. (iii) an equivalent form of documentation; and
   3. (c) an employee report covering the 12 months leading up to application submission.
