1. (1) The review committee and the Superintendent shall make recommendations to the Board based on:

   1. (a)(i) the criteria described in Subsection 53F-5-503(2) for a planning grant;

      1. (ii) the criteria described in Subsection 53F-5-504(2) for an implementation grant; or
      2. (iii) the criteria described in Subsection 53F-5-505(2) for an expansion grant;
   2. (b) the LEA's proposed budget for the LEA's personalized, competency-based learning program; and
   3. (c) the LEA's outcome-based measurements described in Subsection (2).

2. (2)(a) An LEA shall include outcome-based measurements as part of the LEA's personalized, competency-based learning program to measure the performance of the LEA's plan.

   1. (b) The outcome-based measurements described in Subsection (2)(a) shall include at least one measurement of student growth and proficiency.

   2. (c) The outcome-based measurements described in Subsection (2)(a) may include:

      1. (i) parent and student satisfaction with the LEA's personalized, competency-based learning program;
      2. (ii) cost savings;
      3. (iii) an increase in the LEA's graduation rate;
      4. (iv) number of credits earned by students through the personalized, competency-based learning program; and
      5. (v) other quality program indicators as listed in Utah's Personalized Competency-Based Learning Education Framework.

3. (3) An LEA may be awarded each subsequent phase of the grant program only after:

   1. (a) successful participation in the previous phase; and
   2. (b) Board approval of the LEA's plan.

4. (4) If an LEA's grant application is denied by the Board:

   1. (a) an LEA may submit a new grant application the next year following the first denial;

   2. (b) an LEA may only submit a planning grant application the next year following a second denial and the LEA shall:

      1. (i) request technical assistance from the Superintendent prior to re-applying; and
      2. (ii) demonstrate increased understanding of personalized, competency-based learning implementation upon re-application.
